Prohibits physicians from performing, inducing, or attempting to induce an abortion if the probable gestational age of the unborn human being is greater than fifteen weeks, except in medical emergencies.

Requires physicians to determine and document the probable gestational age of the unborn human being according to standard medical practices before performing an abortion.

Mandates physicians performing abortions after fifteen weeks' gestation to file a report with the Department of Health Services within fifteen days, including the method used, gestational age calculation, and medical emergency justification, without identifying the pregnant woman.

Classifies violation of the fifteen-week prohibition as a class 6 felony for physicians and subjects violating physicians to license suspension or revocation and civil penalties up to $10,000 for false or missing reports.

Exempts pregnant women from prosecution for conspiracy to commit violations and preserves existing state abortion restrictions under section 13-3603.
