Amends Section 36-3295 of Arizona Revised Statutes concerning the health care directives registry maintained by a qualifying health information exchange organization.

Expands access rights to allow not only the person who submitted a document and the subject of the document to access health care directives, but also the surrogate of the person who is the subject of the document.

Permits the qualifying health information exchange organization to use, disclose, and make accessible registry information through the health information organization as authorized by Section 36-3805, subsection A.

Maintains confidentiality of registry information except as allowed by state or federal law, and restricts use to purposes prescribed in the article.

Defines "health care provider" for purposes of registry access to include emergency medical service providers, emergency service technicians, and organ procurement organizations maintaining the donor registry.
