Establishes the Alzheimer's Disease and Other Dementias Advisory Council to examine needs of individuals with Alzheimer's disease or dementia, available services, and healthcare provider capacity in Arizona.

Advisory council consists of 11 voting members appointed by the director (including individuals with dementia, family caregivers, facility representatives, medical professionals, researchers, and advocates) and 6 nonvoting state agency representatives.

Voting members serve two-year terms with council members selecting their own chairperson and vice chairperson; council must meet at least quarterly and hold first meeting within 30 days of member appointment.

Council shall develop findings and recommendations on dementia trends, surveillance systems, available resources and services, workforce training requirements, home and community-based services, research, and public awareness and prevention strategies.

Council must submit a state Alzheimer's Plan to the governor and legislature within 18 months of the law's effective date, and annually submit implementation status reports; updated plans required every four years.
