Expands Arizona's immunization reporting system to include adult immunizations in addition to child immunizations by striking language limiting the system to "child" immunizations.

Requires health care professionals to report adult patient immunizations including the patient's name, address, social security number (if not confidential), and date of birth.

Allows the same entities authorized to receive child immunization information to also receive adult immunization information, including health care providers, schools, health insurers, and designated health information organizations.

Maintains confidentiality protections by restricting disclosure of identifying information and making unauthorized disclosure a class 3 misdemeanor.

Preserves patient opt-out rights allowing adults (or parents/guardians for children) to request that immunization information be withheld from the reporting system.
