Establishes an Arizona voter photo identification card issued by the Secretary of State at no charge to registered voters who lack other acceptable forms of identification.

Requires the Arizona voter photo identification card to contain the applicant's full legal name, residence address, date of birth, height, weight, gender, issuance date, and unique identifying number.

Modifies voter identification requirements to accept the new Arizona voter photo identification card as valid identification for voting, along with existing forms such as driver licenses, tribal enrollment cards, military IDs, passports, and government employee IDs.

Makes the Arizona voter photo identification card valid only for voting purposes and requires cardholders to surrender and reapply with updated information if they change their residence address.

Submits the measure to Arizona voters at the next general election for approval.
