Requires plans to notify providers of prior authorization approval or denial within 5 days for urgent health care services and 14 days for non-urgent services after receiving all necessary information.

Requires plans to provide written receipt acknowledgment in the same format as the request was submitted, unless return contact information is not provided.

Deems a prior authorization request automatically granted if the plan fails to meet the response deadlines and notification requirements.

Prohibits plans from rescinding or modifying a granted prior authorization after services are rendered in good faith, except in cases of provider fraud or misrepresentation.
