Prohibits hospital service corporations, medical service corporations, health care services organizations, disability insurers, and group/blanket disability insurers from imposing cost sharing requirements (deductibles, copayments, coinsurance, out-of-pocket maximums) on breast health examinations effective January 1, 2025.

Applies to screening mammograms, diagnostic breast examinations (mammography, MRI, or ultrasound used to evaluate suspected or detected abnormalities), and supplemental breast examinations (additional screening based on personal or family medical history or increased breast cancer risk).

Amends Arizona Revised Statutes in four separate articles to apply the cost-sharing prohibition across all major types of health insurance coverage in the state.

Excludes limited benefit coverage as defined in section 20-1137 from the requirements for disability insurers and group/blanket disability insurers.
