Requires hospital service corporations, medical service corporations, health care services organizations, and disability insurers to cover genetic counseling and germline mutation testing (including BRCA genes) for individuals with personal or family history of cancer effective January 1, 2024.

Mandates primary care providers attempt to identify adult patients with personal or family history of melanoma, breast, ovarian, tubal, peritoneal, endometrial, colorectal, pancreatic or prostate cancer, or ancestry associated with harmful BRCA mutations.

Requires primary care providers to provide or refer patients meeting cancer risk criteria for genetic counseling and germline mutation testing based on nationally recognized clinical standards and familial risk assessment tools.

Defines "BRCA genes" as BRCA 1 or BRCA 2 genes that when mutated increase risk of breast, ovarian, prostate, pancreatic and melanoma cancers, and "germline mutation testing" as inherited genetic mutation testing per evidence-based clinical practice guidelines.

Directs Arizona Department of Health Services and its contractors to provide screening, genetic counseling, genetic testing and germline mutation testing for harmful inherited genetic mutations when medically indicated under the primary care provider requirements.
