Requires voters or their agents who deliver early ballots at polling places, voting centers, or emergency voting centers to provide identification and written attestation to election board workers.

Voters delivering their own early ballots must present identification where their name reasonably appears to match the ballot affidavit.

Agents delivering another person's early ballot must present identification and attest in writing that they are the voter's family member, household member, or caregiver as defined in section 16-1005.

Extends identification and attestation requirements to voting centers and emergency voting centers that accept early ballots.

Makes knowing violation of the new identification and attestation requirements a class 5 felony.
