Prohibits county recorders from using signatures on preprinted third-party request forms (cards, printed documents) as the sole exemplar for subsequent voter signature comparisons; only official county recorder or election official forms qualify as signature exemplars.

Restricts distribution of early ballot request forms to only candidates, political parties, county recorders, or election officials; violations subject to civil penalties up to three times production and distribution costs.

Requires early ballot request forms received by political parties or candidates to be submitted to the political subdivision within six business days of receipt or eleven days before election, whichever is earlier; failure results in civil penalties of up to $25 per day per form or class 6 felony for knowing violations.

Exempts special taxing districts formed under Title 48 for agricultural land or crop protection from early ballot request form distribution and submission requirements.

Updates language throughout sections 16-542 and 16-544 regarding who may distribute election forms and clarifies submission procedures for early ballot and active early voting list request forms.
