Mandates assisted living facilities notify residents' authorized representatives when a resident is transported to a hospital and maintain records of emergency responses for two years.

Requires hospitals to provide written discharge plans to assisted living facilities that include patient medical assessment, pressure injuries, cognitive/physical conditions, weight-bearing status, dietary needs, medication information, and equipment orders.

Establishes that hospitals must maintain available points of contact for 48 hours after discharge to respond to questions from receiving assisted living facilities and coordinate patient screenings before discharge.

Requires hospitals to contact the originating assisted living facility after initial assessment to discuss postdischarge health care needs and allow facility staff to screen patients before discharge to determine if care needs can be met.
