Designates the Arizona Department of Health Services as the lead state agency to address Alzheimer's disease and related forms of dementia.

Establishes a dementia services program within the department to coordinate policies, programs, outreach, research, and federal funding related to Alzheimer's disease and dementia services.

Requires the department to develop and publish an Alzheimer's disease state plan assessing current impact, existing services, gaps, and providing strategic recommendations for increasing access to care, improving quality, and advancing early detection.

Mandates the department review and update the state plan every three years in collaboration with persons with Alzheimer's disease, caregivers, and public and private organizations focused on dementia care and research.

Appropriates $300,000 and two FTE positions from the state general fund in fiscal year 2023-2024 to establish and implement the dementia services program and Alzheimer's disease state plan.
