Removes the restriction that prevented individuals born between June 20, 1968 and September 29, 2021 from obtaining copies of their sealed original birth certificates due to adoption.

Allows adopted individuals who are at least 18 years old and born in Arizona to request a copy of their original birth certificate by submitting a written request to the state registrar.

Requires the state registrar to develop a contact preference form for birth parents to indicate whether they want direct contact, contact through an intermediary, or no contact with the adopted individual.

Allows birth parents to optionally complete a medical history form to be provided to adopted individuals along with their original birth certificate.

Specifies that copies of original birth certificates are not certified copies and cannot be used for legal purposes.
