Removes the date restriction that previously prevented individuals born between June 20, 1968 and September 29, 2021 from accessing their sealed original birth certificates due to adoption.

Allows adopted individuals who are at least 18 years old and born in Arizona to request and receive a copy of their original sealed birth certificate by submitting a written request to the state registrar.

Requires the state registrar to develop a contact preference form for birth parents to indicate whether they want direct contact, contact through an intermediary, or no contact with the adopted individual.

Requires the state registrar to develop a medical history form for birth parents to complete voluntarily, with both forms kept confidential and sealed together with the original birth certificate.

Permits birth parents to file amended contact preference forms or update their information electronically, in writing, or in person with the state registrar.
