Adds a counseling exception to Arizona's parental consent requirement for minors seeking abortion, allowing physicians or counselors to provide specific pregnancy information and counseling as an alternative to obtaining parental consent or judicial authorization.

Requires the counselor or physician to explain that information is objective and non-coercive, discuss all pregnancy alternatives (carrying to term, adoption, foster care, and abortion), and explore involving parents or guardians in the minor's decision-making.

Mandates the minor sign a form documenting receipt of counseling on prenatal/postnatal care, alternatives to abortion, ability to withdraw the abortion decision, birth control information, and discussion of parental involvement.

Requires the physician or counselor to retain a copy of the signed counseling form and transmit it to the minor's attending physician if the counselor is not the attending physician.

Maintains existing penalties for violations (class 1 misdemeanor) and civil remedies for parents or guardians, including damages up to $5,000 or three times the abortion cost, whichever is greater.
