Adds a new exception to Arizona's parental consent requirement for minors seeking abortion when the attending physician receives the minor's informed written consent and determines the minor is mentally and physically competent to give consent under all surrounding circumstances.

Requires the physician to inform the pregnant minor that she is pregnant, the number of weeks of pregnancy, and the particular risks associated with her pregnancy and the abortion technique to be performed.

Maintains existing exceptions to the parental consent requirement for cases involving incest, medical emergencies threatening the minor's life or health, and judicial authorization through court proceedings.

Preserves all other provisions of current law including the 48-hour court hearing requirement, confidentiality protections, civil liability provisions, and statutory damages of $5,000 or three times the cost of the abortion, whichever is greater.
