Expands the immunization reporting system to include adults in addition to children by amending section 36-135, Arizona Revised Statutes.

Requires health care professionals to report adult patient immunization data including name, address, social security number (if not confidential), and date of birth.

Permits health care professionals to submit immunization information to the department weekly or monthly by telephone, fax, mail, computer, or other approved methods.

Maintains confidentiality protections for immunization data with authorization for release only to the patient, health care providers, schools, health plans, and other specified entities with business associate agreements.

Preserves patient right to request withholding of confidential immunization information from all persons, including authorized recipients, prior to receiving an immunization.
