Establishes a transition program allowing the Arizona Department of Corrections to release eligible inmates to community-based transition services for up to 90 days before full release.

Requires the department to contract with private or nonprofit entities to provide services including psychoeducational counseling, case management, substance abuse treatment, anger management, cognitive behavioral therapy, and job placement.

Sets eligibility criteria including minimum/medium custody classification, no sexual offense or violent crime convictions (with exceptions for assault, aggravated assault, and robbery), no felony detainers, and no major rule violations in the current incarceration period.

Mandates the department submit an annual report by July 31 to the governor and legislature detailing recidivism rates, number of inmates served, types of services provided, and waitlist numbers.

Allows a one-time 90-day extension of transition services if the contracted entity identifies the inmate's criminogenic factors using an empirically validated risk assessment instrument and submits an updated service plan.
