Requires school districts and charter schools that provide routine health care services to students to provide parents or guardians with health care credentials of individuals who provide those services upon request.

Defines "health care credentials" as a license, certificate, permit, or registration to practice a health care profession under Arizona Revised Statutes Title 32.

Prohibits schools from disclosing personally identifiable information of health care service providers, including address, salary, social security number, or telephone number.
