Requires hospital service corporations, medical service corporations, health care services organizations, and disability insurers to provide coverage for genetic counseling, genetic testing, and germline mutation testing for cancer risk based on National Comprehensive Cancer Network guidelines, effective January 1, 2025.

Mandates coverage for genetic tests for cancer risk (BRCA genes, Lynch Syndrome genes, and other hereditary cancer syndrome-related genes) when recommended by a health care provider consistent with current NCCN guidelines, with no deductibles, coinsurance, or cost-sharing requirements.

Requires primary care providers to assess adult patients for personal or family history of specific cancers (melanoma, breast, ovarian, tubal, peritoneal, endometrial, colorectal, gastric, pancreatic, or prostate) and provide or refer for genetic counseling and testing if criteria are met.

Directs the Arizona Department of Health Services and its contractors to provide screening, genetic counseling, genetic testing, and germline mutation testing for harmful inherited genetic mutations for AHCCCS members when clinically indicated.

Names the act the "Donna Hicks Act."
