Requires referral agencies to disclose to prospective residents any business relationships, common ownership, or financial ties with assisted living facilities or homes before making referrals.

Requires referral agencies to disclose the amount or good faith estimate of fees that assisted living facilities will pay them, expressed as a dollar amount or percentage of first month charges.

Allows prospective residents to terminate referral agency services at any time by written or electronic notice, after which the agency is not entitled to referral fees unless the facility was specifically identified before termination or the resident communicated with the agency prior to admission.

Requires assisted living facilities to notify referral agencies of resident admissions within fourteen days and to receive written/electronic copies of disclosures and resident acknowledgements before paying referral fees.

Establishes civil penalties of up to $1,000 per violation, enforceable by the attorney general or county attorneys in superior court.
