Requires the Department of Transportation to provide applicants with a statement describing voting eligibility requirements, penalties for false registration, and confidentiality protections, with applicant attestation under penalty of perjury.

Establishes that the county recorder must receive completed registrations from the Department of Transportation within five days and must notify applicants of their ability to decline registration within 21 calendar days.

Provides liability protection for individuals who unknowingly register while not qualified to vote, exempting them from false registration or false swearing charges.
