Amends Arizona Revised Statutes Section 28-669 regarding accident report forms prepared by the Department of Transportation.

Clarifies that written reports required to be made by investigating officers must call for sufficiently detailed information about the cause, conditions, persons, and vehicles involved in traffic accidents.

Requires accident reports to be made on the department-approved form and contain all required information unless such information is unavailable.

Directs the department to modify the written accident report form in conjunction with state and local law enforcement agencies, engineering agencies, and emergency medical service organizations.
