Amends Arizona Revised Statutes section 36-2907 to add a new subsection requiring the Arizona Health Care Cost Containment System (AHCCCS) to provide exceptions to visit limitations for physical therapy services when medically justified.

Exception to physical therapy visit limits applies when documentation shows a member requires continued skilled therapy for their condition.

Clarifies that monies from the hospital assessment fund cannot be used to fund chiropractic services or diabetes outpatient self-management training services under AHCCCS.

Maintains existing coverage rules and limitations for other health and medical services under the state Medicaid program.
