Divides Maricopa County into four separate counties: Hohokam, Maricopa, Mogollon, and O'odham, with specific geographic boundaries defined by city limits, geographic features, and surveyed lines.

Establishes a three-year transition period during which existing Maricopa County operations continue under an intercounty oversight and accountability board chaired by supervisors from each new county.

Requires special elections within 120 days to elect boards of supervisors for the four new counties to staggered, abbreviated terms until the next presidential election; initially elected at-large, then by supervisorial districts thereafter.

Requires Hohokam, Mogollon, and O'odham counties to hold special elections within 120 days to determine county seats through a municipality application process.

Mandates intergovernmental agreements between the four counties for shared services, community college operations (minimum 10 years), and shared capital assets; effective December 31, 2024.
