Amends Arizona law to allow individuals to change the sex designation on their birth certificate by submitting a written request and a statement from a licensed medical or mental health professional confirming they have undergone surgical, hormonal, psychological, or other treatment for gender transition based on contemporary medical standards.

Removes previous requirements that specifically required verification of a sex change operation or chromosomal count, replacing them with broader medical and mental health professional attestation of gender transition treatment.

Applies to persons born in Arizona, with parents or legal guardians able to request amendments on behalf of children.

Maintains existing provisions allowing birth certificate amendments for adoption, paternity acknowledgment, and court-ordered changes.

Requires the state registrar to seal previously registered birth certificates and evidentiary documents when amendments are made, with access only permitted pursuant to court order or existing statute.
