Each public school must provide parents/guardians an annual disclosure document containing the school's letter grade, nearby charter schools, Arizona empowerment scholarship account program overview, and a written notice about educational options and parental rights.

The disclosure document must include prominently formatted sections titled "Your child has options" listing five educational choices (district schools, charter schools, private schools with scholarship accounts, private schools with tuition organizations, and homeschools) and "You have rights as a parent" detailing parental rights regarding education, health care, medical records, biometric scans, DNA records, video/voice recordings, and child safety investigations.

Schools must file the signed and initialed disclosure document in each student's permanent file annually and provide a copy to parents; parents must initial each disclosure section and sign attestation statements.

Schools must assist parents who choose to pursue charter school admission or Arizona empowerment scholarship account participation after receiving the disclosure document.

The Arizona Department of Education shall create a standardized form for the disclosure document that public schools may use to comply with these requirements.
