Mandates behavioral health entities and sober living homes maintain independent third-party statutory agents in the same county, prohibits post office box addresses, and requires notarized declarations for certificate disclosures.

Increases civil penalties for violations from $500 to $5,000-$10,000 per violation (assessed per resident impacted) and makes operating without a license a class 6 felony; prohibits removing licensees or rescinding sanctions without good cause.

Requires sober living homes to maintain 2 paid staff per 6 residents, provide 24/7 on-site supervision, arrange safe transportation for discharged residents, and ensure naloxone is accessible with trained staff; imposes new fingerprinting and criminal background check requirements for employees.
