AZ SB1715

Amends the immunization reporting system to include adult immunizations in addition to child immunizations, expanding the scope from the existing child-only system.

Requires health care professionals to report adult patient immunizations with name, address, social security number (if not confidential), and date of birth, following the same reporting process as child immunizations.

Authorizes the department to release adult immunization identifying information to the same entities currently authorized for child records, including health care providers, schools, health insurance entities, and designated health information exchanges.

Maintains confidentiality protections for adult immunization data, allowing patients to request that their information be withheld from the reporting system by submitting a form before vaccination.

Preserves existing civil and criminal liability protections for good-faith reporting and classifies unauthorized disclosure of confidential information as a class 3 misdemeanor.
