Requires higher education institutions that mandate immunization proof to fully disclose requirements and exemptions to prospective students.

Exempts students age 18 and older from immunization requirements if they submit a signed statement confirming they received information about immunizations and decline due to personal or religious beliefs.

Allows exemption through written certification signed by both student and physician or registered nurse practitioner stating required immunizations may be detrimental to health, valid only for the duration of the medical condition.

Applies to universities under Arizona Board of Regents, community colleges, and career technical education districts serving adult students.

Defines "documentary proof" using the same meaning as section 15-871 of existing law.
