Amends Arizona Revised Statutes Section 15-872 regarding school immunization requirements and documentation.

Requires schools to fully disclose immunization requirements and exemptions to parents, guardians, or persons in loco parentis in any communication about school immunization requirements.

Allows pupils to attend school if they have received at least one dose of each required immunization and have established a schedule for completing remaining doses, with parental presentation of proof and physician-prepared completion schedule.

School administrators must review immunization records for pupils on completion schedules at least twice per school year and suspend pupils who fail to comply with their immunization schedule unless an exemption is provided.

Provides civil immunity to schools and employees for admission, readmission, and suspension decisions made in good faith implementation of immunization requirements.
