Health care insurers must provide a telephone number or email address to reach an individual or department that can explain claim or prior authorization denials and address questions about the denial.

Health care insurers must provide a substantive response to questions about why a claim or prior authorization was denied within two business days of receiving the questions.

The new requirements apply to all claim and prior authorization denials issued by health care insurers.

The law becomes effective June 30, 2026.
