Amends Arizona Revised Statutes sections 8-533, 8-841, and 8-844 relating to child welfare proceedings and parental rights termination.

Requires petitioners in termination of parental rights cases to file a notarized affidavit certifying full disclosure and exchange of all information in the Department of Child Safety's custody and other evidence, including descriptions, dates, and delivery methods of disclosed information.

Requires dependency petitions to include a notarized affidavit from the filing party certifying full disclosure and exchange of all Department of Child Safety information and evidence, with details on what was disclosed, when, and how.

Requires the Department of Child Safety to file a report within six months of a dependency adjudication hearing indicating whether the child continues to meet dependency criteria based on the original petition allegations or if the dependency status or allegations have changed.
