Secretary of State shall develop and maintain a standardized voter registration form for use by all counties, including applicant's name, date of birth, residential address, mailing address (including post office box options), contact information, citizenship affirmation, and signature.

Form must contain clear voter registration deadlines and instructions with consistent formatting, font, and layout across the state as prescribed by the Secretary of State.

Post office box addresses are allowed as mailing addresses but cannot replace the required residential address; Secretary of State shall establish specific formatting guidelines for post office box submissions to ensure uniformity.

All counties must adopt the standardized form and post office box formatting requirements within one year of the effective date; Secretary of State shall conduct regular compliance audits and may take corrective actions.

Secretary of State shall provide guidance, training, and voter education materials to election officials regarding proper use of standardized forms; knowingly providing false information on registration, including misrepresentation of addresses, constitutes false registration under Section 16-182.
