Establishes the Arizona HEALTH Program as a single-payer health care system administered by the Arizona Health Care Cost Containment System, providing comprehensive health benefits to all Arizona residents at no premium or charge.

Creates a 56-member Board of Trustees composed of ex officio directors, 31 gubernatorially-appointed trustees representing consumers, health care providers, labor, employers, and health finance experts, plus trustees appointed by legislative leadership.

Requires the board to develop a benefit coverage plan including all services covered by AHCCCS, Arizona's long-term care system, CHIP, state employee health benefits, ASRS retiree plans, and Medicare within 12 months.

Mandates a revenue proposal including a progressive payroll tax (employer pays minimum 80%, employee 20%), progressive income tax on other income sources with $25,000 exemptions, and $50,000 exemptions for Medicare enrollees.

Prohibits the administration from contracting with health insurers or third-party administrators; requires legislature to adopt the revenue proposal before program implementation.
