Requires voters or their agents who drop off voted early ballots at polling places, voting centers, or emergency voting centers to present identification and complete written attestation.

Specifies that voters delivering their own early ballots must present identification matching their name on the ballot affidavit as required by section 16-579.

Requires a voter's agent delivering another person's ballot to present identification and attest in writing that the agent is the voter's family member, household member, or caregiver as defined in section 16-1005.

Makes it a class 5 felony to knowingly violate the identification and attestation requirements for early ballot delivery.

Applies identification and attestation requirements to voted early ballots accepted at voting centers and emergency voting centers established under section 16-411.
