Divides Maricopa County into four separate counties: Maricopa, Hohokam, Mogollon, and O'odham, each with defined geographic boundaries based on existing survey lines and city/town boundaries.

Establishes a three-year transition period during which the current Maricopa County Board of Supervisors retains authority, with an intercounty oversight and accountability board composed of each new county's board chairperson making shared decisions.

Requires special elections within 120 days to elect supervisors for the four new counties on staggered, abbreviated terms; currently elected Maricopa supervisors continue their terms in the new county where their district is located.

Each new county (Hohokam, Mogollon, and O'odham) shall hold a special election within 120 days after supervisor elections to select a county seat from applications submitted by municipalities.

Requires the four counties to enter into intergovernmental agreements for continued operation of community colleges for at least ten years and for management of special districts; counties may share capital assets under a ten-year shared use agreement.
