Requires hospitals accepting Medicaid payments to include a question on patient admission/registration forms asking whether patients are U.S. citizens, lawfully present, or not lawfully present in the United States.

Mandates hospitals submit quarterly reports to the Department of Health within 30 days after each calendar quarter showing the number of admissions and emergency department visits categorized by patient immigration status or declined responses.

Requires the Department of Health to submit an annual report to the governor, legislature, and secretary of state by March 1 each year summarizing hospital admissions and emergency visits by patient immigration status for the prior calendar year.

Annual departmental report must include information on uncompensated care costs for undocumented aliens, the impact of uncompensated care on hospital services and funding, and related information.

Prohibits rules or disclosures that require hospitals to report patient names or personal identifying information to the department; includes a statement on forms that responses will not affect care or trigger immigration authority reports.
