Amends Arizona Revised Statutes section 20-3117 to clarify the state's surprise billing dispute resolution process.

Requires health insurers to include notice of enrollee rights in explanation of benefits statements for claims involving noncontracted health care providers, excluding claims subject to federal No Surprises Act dispute resolution.

Mandates health care providers, their representatives, and billing companies provide written notice of the dispute resolution process when contacted about surprise out-of-network bills not covered by the No Surprises Act.

Requires the Department of Insurance to post consumer information on its website explaining what constitutes surprise bills, how to avoid them, and how to use the dispute resolution process.

Effective date: May 2, 2025 (approved and filed).
