Amends Arizona's immunization reporting system to include adults in addition to children, expanding the scope from a child-specific system to cover all patients receiving immunizations.

Requires health care professionals to report adult patient immunization data including name, address, social security number (if not confidential), and date of birth, in addition to existing vaccine type and date information.

Maintains confidentiality protections for adult immunization records with the same restrictions on disclosure as child records, limiting release to patients, health care providers, schools, and other authorized entities.

Allows adults or their health care decision makers to request that their immunization information be withheld from the department reporting system by submitting a signed form before vaccination.

Preserves existing penalties for health care providers who fail to comply with reporting requirements and for unauthorized disclosure of confidential immunization information.
