Requires the AHCCCS director to annually submit waiver applications to the Centers for Medicare and Medicaid Services by March 30 to authorize payments for covered services provided to American Indian and Alaskan Native members at Indian Health Service and tribal facilities.

Targets restoration of services that were eliminated, reduced, or limited from the state Medicaid plan on or after September 2010, including medically necessary diagnostic, therapeutic, and preventative dental services.

Applies specifically to participating Indian Health Services facilities and facilities operated by tribes or tribal organizations under the Indian Self-Determination and Education Assistance Act (P.L. 93-638).

Authorizes AHCCCS to make payments for these expanded services upon federal approval, with the director required to continue submitting annual waiver applications until all requested waivers are approved and in effect.
