Requires health care insurers to provide a telephone number or email address for a department that can explain why a claim was denied and answer related questions.

Mandates insurers provide a substantive response to questions about claim denials within two business days after receiving the questions.

Establishes identical requirements for prior authorization denials, including contact information and a two-business-day response deadline for questions.

Adds two new sections (20-3104 and 20-3105) to Arizona Revised Statutes Title 20, Chapter 20, Article 1 regarding timely payment of health care insurance claims.

Takes effect June 30, 2027.
