Limits the Arizona Department of Health Services' access to nursing care institution personnel records to specific information including employee names, contact details, employment dates, qualifications, licensure, and required training documentation.

Requires the department to issue statements of deficiencies to nursing care institutions within ten business days after completing state surveys.

Prohibits the department from initiating complaint investigations against nursing care institutions more than twelve months after the alleged incident occurred, unless the complaint involves allegations of criminal abuse or neglect with failure to report.

Mandates complaint investigations be initiated consistent with Centers for Medicare and Medicaid Services time frames based on the severity level of each complaint.

Establishes that long-term care incident reports are confidential internal risk management tools used for quality improvement, maintained separately from resident medical records.
