Creates a voluntary prohibited possessor program allowing Arizona residents to request their own name be added to a list restricting them from purchasing or possessing firearms for up to 180 days.

Individuals can submit the request form through a law enforcement agency (after identity verification) or through their health care provider, who then forwards it to the court.

The court must add the person's firearm ineligibility to the National Instant Criminal Background Check System and maintain the voluntary prohibited possessor list.

After a minimum of 30 days, individuals may request removal from the list; automatic removal occurs at 180 days unless the person voluntarily extends the restriction period.

All records related to a person's participation in the voluntary prohibited possessor program must be destroyed within 5 days of their removal from the list.
