Requires the Arizona Department of Education to post on its website and include in each Empowerment Scholarship Account (ESA) application packet a notice outlining all legal rights that students and parents waive by enrolling in the ESA program.

Applies to Arizona's school voucher program, which provides public funds for private school tuition and other educational expenses.

Amends Section 15-2403 of the Arizona Revised Statutes regarding ESA administration.

Maintains existing ESA application timeline (July 1 through June 30) and 30-day award letter requirement, while adding the new disclosure requirement.
