Requires health insurers, HMOs, and disability insurers to cover genetic counseling, genetic testing, and germline mutation testing for cancer risk based on National Comprehensive Cancer Network guidelines, effective January 1, 2027.

Coverage applies to individuals with personal or family history of cancer or inherited gene mutations, including BRCA1/BRCA2 (breast cancer genes) and Lynch Syndrome genes (MLH1, MSH2, MSH6, PMS2, EPCAM).

Mandates that all covered genetic testing and counseling be provided without deductibles, coinsurance, or other cost-sharing requirements.

Requires primary care providers to screen adult patients for hereditary cancer risk factors and refer at-risk patients for genetic counseling and testing when clinically indicated.

Extends coverage requirements to Arizona's Medicaid program (AHCCCS) to the extent authorized by federal law, and may be cited as the "Donna Hicks and Clarissa Amparano Act."
