Establishes a home and community-based services program for adults with serious mental illness through the Arizona Health Care Cost Containment System (AHCCCS), requiring federal approval by July 1, 2027 with implementation within one year of approval.

Limits initial program enrollment to 500 members, with priority given to individuals under court-ordered treatment, those with legal guardianship due to psychiatric incapacity, people discharged from jails, prisons, or state hospitals, and homeless individuals.

Sets financial eligibility at income not exceeding 300% of the federal Supplemental Security Income benefit rate, with qualified income trusts available for those exceeding this threshold.

Covered services include attendant care, habilitation with behavior management, supervised community living (including assisted living-type supports), respite care, home-delivered meals, nursing and medication administration services, and non-emergency transportation.

Requires providers to implement eviction-prevention protocols and obtain regional behavioral health authority approval before issuing non-emergency notices to vacate, with annual reporting to the legislature on enrollment, housing stability, hospitalizations, and program costs.
