Requires insurers offering Medicare supplement insurance to persons 65+ to also offer policies to individuals under 65 who qualify for Medicare due to end-stage renal disease (ESRD) or amyotrophic lateral sclerosis (ALS)

Establishes enrollment periods for under-65 Medicare enrollees with ESRD or ALS, including within 6 months of enrolling in Medicare Part B or January 1, 2027, whichever is later

Creates a special enrollment window from December 2, 2025 through June 1, 2027 for individuals under 65 already enrolled in Medicare Part B due to ESRD or ALS

Prohibits insurers from charging enrollees under 65 with ESRD or ALS premium rates exceeding those charged to enrollees who are 65 years of age for the same plan

Mandates that all benefits and coverages available to Medicare enrollees 65+ must also be available to under-65 enrollees who qualify due to ESRD or ALS
