Mandates coverage include at least three plan years of storage for preserved reproductive materials, with no continuing obligation if the insured changes plans during that period.

Requires insurers to process prior authorization requests for fertility preservation services as urgent, responding within 72 hours.

Allows religious employers to request and receive exemptions from coverage requirements if they conflict with sincerely held religious beliefs, with written notice required to prospective enrollees.

Applies to hospital service corporations, medical service corporations, health care services organizations, and disability insurers across multiple sections of Arizona insurance law.
