Applies to accident and health insurers, hospital and medical service corporations, and health care services organizations regulated under Arizona insurance law.

Coverage must be provided in the same manner as other reconstructive surgery benefits, preventing insurers from treating autologous reconstruction differently than implant-based reconstruction.

Amends multiple sections of Arizona Revised Statutes (Title 20) related to insurance coverage requirements for breast reconstruction surgery.

Effective date is January 1, 2026, giving insurers time to update their policies and coverage offerings.
